Service Coordination
Service Coordination begins when an individual requests assistance in accessing medical, social, educational, and other appropriate services and supports and it continues throughout his/her relationship with the Center. The plan of services and supports is based on a person-directed process that is consistent with the DADS’s Person Directed and Family Directed Guidelines.

Intellectual and Developmental Disabilities Community Services
Community Living Skills
Community Living Skills assists individuals with severe or profound developmental disabilities. This service offers necessary skills training in the areas of motor, communicative, cognitive, personal hygiene, and social development.

Community Support Services
Individualized activities that are consistent with the individual’s person-directed plan and are provided in the individual’s home and at community locations. Supports include habilitation and support activities, activities for the individual’s family that help preserve the family unit, transportation for an individual between home and the individual’s community employment site or day habilitation site; and transportation to facilitate the individual’s employment opportunities and participation in community activities.

Job Quest
Job Quest provides employment assistance to an individual in locating paid, individualized, competitive employment in the community. Once employment is obtained, assistance to help the individual sustain that employment is provided. It includes individualized support services consistent with the individual’s person-directed plan as well as supervision and training. Competitive employment is at a pay scale no less than minimum wage.

Ticket to Work and Self Sufficiency Program
The goal of the Ticket to Work and Self Sufficiency Program is to increase opportunities and choice for Social Security disability beneficiaries to obtain employment. Staff provide beneficiaries with employment assistance and/or job coaching/support to obtain and maintain employment. This service is administered by the Social Security Administration (SSA).

Transition Services
Transition Services are provided to area high schools’ Life Skills graduating students to develop work skills and to develop school to work plans. The transition team works cooperatively with the schools to educate students, parents and school personnel on the philosophy of Supported Employment.

Recreation
Recreation provides a variety of opportunities for individuals with intellectual and developmental disabilities to participate in community recreational/leisure activities and Special Olympics. Cardiovascular fitness and skills acquisition are also offered. An after-school program and summer day camp are available to special education students within the public school system and may be provided in conjunction with the Arc of Greater Beaumont.

Vocational Training
Vocational Training provides opportunities for individuals, sixteen and older, with intellectual and developmental disabilities to gain work habits necessary to secure and maintain competitive employment. This training is provided through in-house contracts, enclaves, mobile crews and vocational client work positions in conjunction with personal-social adjustment training.

Intellectual and Developmental Disabilities Clinical Services
Communication Therapy
Communication Therapy provides services to individuals with hearing and language impairments. Assessments are conducted to determine communication deficits. Communication skills are enhanced using a variety of adaptive equipment such as computers and other assistive devices.

LEAP (Living Environment Adapted for People)
LEAP serves adults 22 and older who have completed high school. The program is designed to serve people whose physical limitations preclude independence in activities of daily living. Individuals who are appropriate for the program demonstrate significant impairments in mobility and/or communication.

Autism Services
Autism services provides instruction based on methods directly related to autism. Individualized training is provided on new tasks and the opportunity to generalize skills into the home, community and other settings. Individuals will benefit from routines and structure in order to thrive and flourish. The management of classroom environment and visual supports is an important way to politely impact behavior. A Structured Learning Environment assists in reducing anxiety, increasing independence and managing behavior.

The Home and Community-Based Services Program is a Medicaid Waiver Program authorized under Section 1915(c) of Title XIX of the Social Security Act. The program provides an array of services to support individuals to live in their family home, their own home, a foster care home or group home and assist with accessing care, activities and events that are available to all members of the community.

Intermediate Care Facility/Mental Retardation (ICF/MR)
ICF-MR Programs are 24 hour residential settings that provide a variety of services emphasizing active treatment in assisting residents increase independent living skills. A comprehensive functional assessment is completed for each resident to identify area of strengths and needs and guide the development of the Person Directed Plan. The Tenth Street House serves six (6) individuals in Jefferson County. The JoAnn Fries House serves eight (8) individuals in Hardin County.

Respite Care
Respite Care is planned or emergency short termed relief services provided to an individual’s unpaid caregiver when the caregiver is temporarily unavailable to provide supports due to non-routine circumstances. This service provides individuals with intellectual and developmental disabilities with personal assistance in daily living activities and functional living tasks. Respite Care is provided either in the individual’s home or away from the home.

In-Home and Family Support Program - MR
The IHFS Program-MR is a grant program authorized by the Texas Health and Safety code, Chapter 535 that is administered by the LA. The program is designed to provide funds to individuals, or their families, for the purchase of supported living services and/or goods that are not available through any other resource. Items requested must be specifically linked to the person’s disability, either supporting the recipient in his or her own home or assisting the family to maintain the person in the family home.
